1. Is it a local installation (i.e. in your own computer) or in a shared, virtual or dedicated server?
2. Is it in a Windows or a Linux environment?
3. Did you verify that your environment (php, mysql) complies with the minimum requirements, as mentioned in the download page?
4. What method did you follow to do the installation: manually, via an automatic script (e.g. softaculous) or with Git?
5. Did you get any errors during the installation?
6. Once installed, did you change any settings or you just started to create your course?
7. What is the "certain" information your are changing from the user profile that gives you the database error?
Please also enable the debug system.
