The main confusion is that there is no "Migration-Upgrade". That "open" format http://en.wikipedia.org/wiki/Office_Open_XML from a closed company is just additional pain.

There is http://docs.moodle.org/en/Moodle_migration and there is http://docs.moodle.org/en/Upgrading. "Migration-Upgrade" is something new. Not that it is wrong, but you must know what you are doing.
The prudent advice is the one-step-at-a-time approach, fixing each step in case you need to retract. For example:
Step 1. http://docs.moodle.org/19/en/Moodle_migration from 1.9.X to 1.9.X in the new server. (You copy the Moodle code from the old server to the new server)
Step 2. http://docs.moodle.org/19/en/Upgrading the 1.9.X in the new server to 1.9.latest
Step 3. http://docs.moodle.org/22/en/Upgrading_to_Moodle_2.2 your 1.9.latest to 2.2.latest
Step 4. http://docs.moodle.org/23/en/Upgrading_to_Moodle_2.3 your 2.2.latest to 2.3.latest (it need not be 2.3, you can go straight to 2.4 or even 2.5 if you are adventurous)
