We recently upgraded from 2.2 to 2.5 (completely separate installation). Course files were backed up in 2.2, then imported into 2.5 with no apparent problems (well, a couple, but that's another story).
Here's the oddity: One teacher still receives the separate "Add a resource..." and "Add an activity..." menus on her course pages (two courses). When other admins for her log in to her course, we get the combined "Add a resource or activity..." menu.
We have tried different machines, different web browsers, and the one teacher continues to get the old menus.
We finally deleted her entire account, created a new one (different middle initial), and made her a teacher in her courses. Now, everything works as it should.
Anyone else have this problem? It seems that there should be no way this should happen, but it did. We've solved the problem, but wanted to share the experience.
